Eligibility: Open to U.S. residents where permitted by law.

These terms govern the "Share & Win Summer Fun" referral campaign run inside PbN Advantage (PbNA), the Practice by Numbers customer loyalty program.

1. Overview

Share & Win Summer Fun is a referral sweepstakes for active PbN Advantage customers. Customers who refer other dental practices to PbN receive raffle entries, PbN Advantage Points, and a Visa® Gift Card reward if a referred practice becomes a paying PbN customer. PbN holds monthly prize drawings during the campaign window.

2. Who can participate

To enter, a person must be:

An active PbN customer.
Logged into PbN Advantage during the campaign window.
The person actually submitting referral names (not someone submitting on another customer's behalf without permission).
Only PbN customers can participate. Non-customers, vendors, contractors, and PbN employees are not eligible to win raffle prizes.

3. How to submit a referral

Open the PbN app and log into PbN Advantage.
Click the Refer tab in the top navigation.
Complete the referral submission form for each practice you want to refer.
Submit the completed form.
Each customer may submit up to 10 referral names per month during the campaign.

4. Eligibility: what counts as a valid referral

A submitted referral counts toward raffle entries and points only if it meets all of the following:

Dental practice: The referred entity must be a dental practice (general or specialty).
PMS match: The referred practice must use a practice management system (PMS) that PbN supports. PbN will confirm this when reviewing the referral.
No recent demo: The referred practice must not have taken a PbN demo in the last 6 months.
No duplicates or active opportunities: The referred practice must not already exist as a duplicate lead or active opportunity in PbN's CRM.
Not a current PbN customer: The referred practice must not already be a PbN customer.
PbN reserves the right to validate each submission. Submissions that do not meet eligibility will not count as raffle entries or qualify for points.

5. What customers receive

5.1 50 PbN Advantage Points + 1 raffle entry (per eligible referral)

For each eligible referral a customer submits during the campaign:

50 PbN Advantage Points are credited to the customer's PbNA account.
1 entry into that month's raffle drawing is added to the customer's name.
There is a 10-referral monthly submission limit.

5.2 Monthly raffle prize

Each month during the campaign, one winner is drawn at random from eligible entries submitted that month, up to $1,000. The actual prize value is determined by total community referral participation that month, as follows:

0 to 49 - $250 Visa® Gift Card
50 to 99 - $500 Visa® Gift Card
100 or more - $1,000 Visa® Gift Card

5.3 Conversion reward (when a referred practice becomes a customer)

If a referred practice becomes a paying PbN customer within 30 calendar days of the original referral submission date:

1,000 PbN Advantage Points are credited to the referring customer's PbNA account.
A $250 Visa® Gift Card is sent to the referring customer.

Annual cap: The conversion reward is limited to a maximum of two paid conversion rewards per referring customer per calendar year. Any conversion rewards beyond two in a calendar year require the referring customer to complete a W-9 form before payout, per U.S. tax reporting requirements.

6. Drawings, winner selection, and notification

Drawings are conducted by PbN using a random selection method.
Only entries credited through eligible referrals submitted that month are counted.
Drawings are not weighted by customer tenure, plan, or any other factor.
Winners are contacted by email at the address associated with their PbN Advantage account.
If a winner does not respond within 7 business days of notification, an alternate winner will be drawn.

7. Payouts

Raffle prize: Visa® Gift Card, or the equivalent, delivered electronically within 30 calendar days of confirmed winner notification.

Conversion reward: 1,000 PbN Advantage Points credited within 30 business days of the referred practice's PbN start date. $250 Visa® Gift Card delivered electronically within 90 calendar days of the start date.

Annual W-9 trigger: any customer reaching 3 or more conversion rewards in a single calendar year must complete a W-9 before an additional reward is paid.

8. Use of information

By submitting a referral, the referring customer confirms:

They have a reasonable basis to believe the referred practice may be interested in PbN.
PbN may contact the referred practice using the information submitted.
PbN may use submitted information per its Privacy Policy at practicebynumbers.com/privacy.
Referred practices that do not wish to be contacted can request removal by unsubscribing from emails received from PbN.

9. Modifications and right to cancel

PbN reserves the right to:

Change campaign mechanics, dates, or prize structure with reasonable advance notice.
Disqualify any submission found to be fraudulent, duplicative, or in violation of these terms.
End or suspend the campaign early if necessary.

10. Tax responsibility

Prize and conversion reward recipients are responsible for any taxes owed on the value of the prize or reward. PbN will issue applicable U.S. tax forms (W-9, 1099) where legally required.

The following standard sweepstakes terms apply: no purchase necessary, void where prohibited by law, governing law per the laws of the United States, and dispute resolution per PbN's standard terms of service.
