Eaglesoft users can now access a new Medical History Form in Practice by Numbers (PbN) that closely resembles the original Eaglesoft form. This new form is designed to streamline the process of updating the patient’s chart with medical history information.

Key Features of the Form:

  • Automatic Alerts: Items selected from the Eaglesoft list blocks on the form will automatically generate alerts in the patient’s chart. These alerts ensure that critical medical information is easily accessible.

  • Form-Only Fields: Other fields that are filled out on the form will remain within the form itself and will not create alerts.

Below is an example of a completed form. Items highlighted in red indicate the fields that will trigger alerts in the patient’s chart.

Patient Chart Alerts:

The corresponding alerts generated by the selected items on the Medical History Form will appear in the patient’s chart, as shown in the example above.

This integration makes it easier to keep patient records up-to-date and ensures that important medical information is clearly communicated within the practice.
