Eaglesoft users can now access the Medical History Form directly within Practice by Numbers (PbN), where it maintains the same structure as the original Eaglesoft form. This seamless integration allows practices to efficiently update a patient's medical history while ensuring that critical information is readily available in their chart.

Key Features of the Form

Automatic Alerts

Items selected from the Eaglesoft list blocks on the form will automatically generate alerts in the patient’s chart. These alerts ensure that critical medical information is easily accessible.

Form-Only Fields

Other fields filled out on the form will remain within the form itself and will not create alerts.

Editing the Eaglesoft Medical History Form

The Eaglesoft Medical History form within PbN is an Eaglesoft form displayed in the Practice by Numbers interface. This form cannot be edited within PbN and comes directly from what is entered in Eaglesoft. Practices that need to make changes to the form must do so directly within Eaglesoft. The updated form will then reflect in PbN automatically.

Below is an example of a completed form. Items highlighted in red indicate the fields triggering alerts in the patient’s chart.
