How to Use the Eaglesoft Medical History Form

If your practice uses Eaglesoft as your PMS, you benefit from a direct integration that allows you to access your Eaglesoft Medical History Form within Practice by Numbers (PbN). This ensures continuity between your charting tools and allows your team to view patient health data in one unified interface.

Rather than creating a duplicate or separate form, PbN reflects the original Eaglesoft Medical History Form, mirroring its structure, behavior, and field logic. This alignment ensures that any updates made in Eaglesoft are accurately displayed within PbN, improving efficiency and reducing charting inconsistencies.


How the Integration Works

Practice by Numbers automatically pulls Medical History Forms directly from your Eaglesoft database. This includes:

  • The active version of the form.

  • The default form, if one is designated in Eaglesoft (recommended).

These forms are not editable within PbN—they are simply a visual representation of what exists in Eaglesoft. All management and configuration of the form, including its structure and logic, must be done within Eaglesoft.


Common Errors & Troubleshooting

“Form is Not Eligible” Error

This error means the form has not been properly configured in Eaglesoft. To resolve it:

  1. Open Eaglesoft.

  2. Ensure the Medical History Form is marked Active.

  3. (Recommended) Set the form as Default to ensure it appears automatically in PbN.

PbN reads form settings directly from Eaglesoft. If the form isn't correctly configured, it won’t appear or function properly in PbN.

Missing Medical History Form in PbN

If the form disappears from PbN:

  1. Check Eaglesoft to verify the form is still marked Active.

  2. If changes were recently made in Eaglesoft, a re-sync may be required.

  3. In PbN, go to the Form Settings and manually Reactivate the form.

  4. Restart PbN if the form does not reappear after making updates.

Completed Forms Not Syncing Back to Eaglesoft

If completed forms in PbN are not updating in Eaglesoft:

  • Make sure the form is set as Default in Eaglesoft.

  • Only the default form is eligible for syncing back completed data.

  • Verify this setting anytime forms are updated or replaced.

Automatic Chart Alerts

When a user selects a condition, allergy, or medication from the Eaglesoft list blocks on the form, alerts are automatically created in the patient’s chart. These alerts help staff quickly identify critical medical flags.

Example: Selecting “Latex Allergy” on the form will trigger a chart alert in PbN.

Form-Only Fields

Some fields—such as notes or free-text responses—remain within the form only and do not trigger chart alerts. They are still visible when reviewing the form but will not appear elsewhere in the patient record.

Editing the Medical History Form

The Eaglesoft Medical History Form shown in PbN is read-only.

  • You cannot edit the layout or fields from within PbN.

  • All updates must be made using Eaglesoft’s form editor.

  • Once changes are made in Eaglesoft, the updated form will automatically sync into PbN on the next data pull.
