The Multi-Image Upload block in Practice by Numbers Forms makes it easier to collect multiple related images from patients in one place.
Instead of adding several separate image fields to a form, you can add a single Multi-Image Upload block and allow patients to submit multiple images within that field.
Single Image Upload vs. Multi-Image Upload
There are now two image upload options available when building a form:
Single Image Upload allows a patient to upload one image. This is the existing Image Upload block, which has simply been renamed for clarity. Its functionality has not changed.
Multi-Image Upload allows a patient to upload multiple images within the same form field.
Use Multi-Image Upload when you want patients to provide several related photos, such as multiple insurance card images, treatment photos, identification images, or other supporting documentation.
Adding a Multi-Image Upload Block
When creating or editing a form:
Open the form in the Forms editor.
Add a new block to your form.
Select Multi-Image Upload.
Configure the question or instructions you want displayed to the patient.
Save your changes.
The Multi-Image Upload block will appear as a single field on the form, but patients can attach multiple images to it.
How Patients Upload Images
When completing the form, patients can select multiple images at once when their device or browser supports it.
They can also upload images and then add additional images to the same field afterward.
Each uploaded image follows the same image validation requirements used for Single Image Upload fields.
Multi-Image Uploads and Default Data Sources
The Multi-Image Upload block does not support Default Data Sources.
Because PMS image write-back fields expect a single image, Multi-Image Upload fields cannot be mapped to patient.submission.photo or another Default Data Source.
As a result, images collected through a Multi-Image Upload block:
Do not participate in PMS image write-back.
Cannot be configured with a Default Data Source.
Are collected as part of the patient's form submission.
Are included in the completed form PDF.
If you need an uploaded image to write back to a supported field in your PMS, continue using the Single Image Upload block with the appropriate Default Data Source.
How Multiple Images Appear on Completed PDFs
All images submitted through a Multi-Image Upload block are included when the completed form PDF is generated.
The images appear sequentially beneath the associated question or field and maintain the order in which they were uploaded. Images are sized appropriately so they remain clearly visible within the completed form.
When Should I Use Multi-Image Upload?
Multi-Image Upload is useful anytime you want patients to provide several related images without creating a separate form field for every photo.
For example, instead of adding several Single Image Upload blocks labeled Photo 1, Photo 2, and Photo 3, you can create one Multi-Image Upload field and allow the patient to attach all of the necessary images there.
This keeps forms shorter, easier to complete, and easier for your team to review.
