Form Blocks

Here’s a breakdown of all the available building blocks in the PbN Forms Editor, categorized by their purpose:

Section & Instruction Blocks

  • Header: Add a section or form title with optional instructions.

  • Selection Header: Use to introduce grouped multiple-choice questions.

  • Paragraph: Provide additional context, descriptions, or instructions.

  • Image: Insert visual elements (e.g., logos, illustrations).

Input Fields

  • Text Box: For short free-text answers (e.g., names, IDs).

  • Number Box: For numeric inputs like age or ZIP code (with validation).

  • Text Area: For longer responses such as patient notes or descriptions.

  • Select (Dropdown): Choose from a predefined list; supports multi-select and “Other” with free text.

  • Checkbox: Use for consent or agreements with multiple selection options.

  • Radio Buttons: Present mutually exclusive options—only one can be selected.

  • Yes/No: A simple binary response option for fast decisions or confirmations.

  • Date Picker: Select a date via a calendar interface—ideal for birthdates or appointment dates.

Patient-Specific & Medical Info Blocks

  • Medications / Allergies / Diseases / Referral: Pull options from the active lists in Forms → List to standardize medical history input.

  • Patient Name / Gender / Date of Birth / Patient Profile: Collect key demographics and profile data.

Legal & Signature Blocks

  • Terms: Display terms & conditions or consent language for user acknowledgment.
