The Forms > Settings page allows your practice to establish default document storage locations and manage other general Online Forms settings.

Access Forms Settings
Navigate to Forms.
Select Settings.
Locate the setting you want to configure.
Important: Settings are organized into individual sections. Use the applicable Save button after making changes.
Common Settings
The Common Settings section contains options that apply more broadly to the Online Forms experience.
Show Doctor and Witness Signatures in Kiosk App
Enable Show Doctor and Witness Signatures in Kiosk App when doctor and witness signatures should be displayed as part of the applicable Kiosk App workflow.
After changing this option, click Save.
Online Form Settings
Use Online Form Settings to select the default folder or document category where completed general forms should be stored in your Practice Management System (PMS).
Select a Default Folder or Category
Choose the appropriate folder or document category from the available options.
For example, a practice may use a category such as: Patient Identification
The available options depend on how documents are organized within your PMS.
Customize Individual Forms
The selection made under Online Form Settings acts as the default.
If an individual form needs to be stored in a different location, you can customize its folder or document category separately.
This allows your practice to establish a standard storage location without requiring every form to use the same category.
Recommended Workflow
Determine the standard PMS category for your general patient forms.
Select that category under Online Form Settings.
Click Save.
Review individual forms that may require a different storage location.
Configure those forms separately when necessary.
Scanned Document Settings
Use Scanned Document Settings to choose the default folder or document category where scanned or manually uploaded patient documents should be stored.
Select the appropriate PMS category and click Save.
Using a consistent storage location can help team members locate patient documents more easily.
If your PMS document structure changes, revisit this setting to make sure the correct category is still selected.
Reset General Forms
Use Reset general forms when you need to reset general forms that were synced from admin.
Because resetting may remove changes made to synced forms, use this option carefully.
Before resetting:
Confirm that you are working with the correct form type.
Determine whether your practice has customized the synced forms.
Confirm that those customizations no longer need to be retained.
Select Reset only when necessary.
Reset Consent Forms
Use Reset consent forms to reset consent forms that were synced from admin.
This is different from changing the settings that control how consent forms are stored or displayed.
Use this reset option carefully if your practice has customized its synced consent forms.
Reset Admin Package Forms
Use Reset admin package forms to reset forms that were synced through admin packages.
Before using this option, verify whether your practice has made changes to the applicable forms.
Reset Templates
Use Reset Templates when email or SMS templates need to be returned to their built-in/default versions.
This can be useful if a template has been heavily customized and your practice wants to return to the original content.
Important: Resetting a template can remove your existing customizations. Review your current template before resetting.
Use the available Reset and Save controls as appropriate for the section.
Package Description
The Package Description allows your practice to provide patient-facing instructions or other information alongside a package of forms.
This can help patients understand what they need to complete before their appointment.
For example: "The listed forms are required before your appointment. Some forms can only be completed within 12 hours of your appointment time."
Use the Rich Text Editor
The Package Description includes a Rich Text Editor that lets you customize how your message is presented.
Use formatting when it helps make instructions easier for patients to understand. Avoid adding unnecessary information that could distract patients from the forms they need to complete.
Language Options
The Package Description area supports language-specific content.
Use the available language options to review or configure package instructions for the languages supported by your practice's workflow.
When maintaining multiple versions, make sure the instructions communicate the same essential information across languages.
What Should the Package Description Include?
Consider including:
Why the forms are being requested.
Whether they should be completed before the appointment.
Any important timing requirements.
General instructions that apply to the entire form package.
Other information patients should know before beginning.
Information that applies only to one individual form is generally better placed within that form.
Save Your Changes
After making changes to a settings section, click the applicable Save button.
Because different areas of the page have separate Save controls, saving one section does not necessarily mean changes in another section have also been saved.
Best Practices
Use consistent PMS categories for patient documents.
Review storage settings when your PMS document organization changes.
Keep patient-facing package instructions concise.
Review multilingual content when updating Package Descriptions.
Use reset tools carefully.
Confirm that customizations are no longer needed before resetting forms or templates.
Save each section after making changes.
