Displaying Patient Name for New Patient Submitted Forms

When a new or unknown patient submits a form, the system will now display their name along with two key pieces of information:

• An identifier indicating the patient is new (NP).
• The patient's date of birth (DoB).

This information will appear in the following format: John Doe (NP, DoB 07/25)

How It Works

1. Form Submission: When a patient submits a form, the system captures their name and date of birth.
2. Display Information: The patient's name is displayed along with the identifier "NP" to indicate they are a new patient and their date of birth.
