How to Add a New Tablet Connection for the Online Forms Kiosk in PbN


You can use almost any tablet as a kiosk for patient forms, as long as it has an internet connection, but an iPad is highly recommended. Amazon tablets are not recommended as functionality is not as stable. 


Follow these steps to connect a new device:


1. Access the Forms Page:

  • Go to the PbN website.
  • Select the "Forms" page at the top of the website.


2. Add a New Tablet:

  • On the left side, select the "Tablets" option.
  • Click on "Add a new tablet."


3. Name the Device:

  • Enter a unique name for the device. The name must be unique across all accounts. A format like "{Practice Name} Tablet One" is recommended.
  • Note: This name will be visible to patients using the tablet.


4. Pair the Device:

  • After entering a valid name, a PIN number will be provided.
  • On the tablet, go to https://app.practicenumbers.com/forms and enter the PIN to pair the device.
  • Alternatively, you can add a device using the Kiosk app.
    • The Kiosk app is available only for iOS devices.
    • You can download the app directly by scanning the QR code.




5. Set Up the Kiosk:

  • Once paired, go to the same URL on the tablet or the Kiosk App to access the check-in menu for patients.


6. Manage Your Devices:

  • On the "Tablets" page in PbN, you can see all your connected devices.
  • From this page, you can view the connection status, edit the device name, get the 4-digit access pin by selecting the pencil icon, and remove an existing device.
