If your patient needs to complete paperwork or sign a consent form, you can easily send these documents through Practice by Numbers (PbN). This can be done via the Communication Center or the Patient Information Window.
Sending Forms via the Communication Center
Open the Communication Center:
Locate the SMS button at the bottom left of any page to access the Communication Center.
Select the Patient:
The Communication Center displays recent conversations. To start a new message, click the pencil icon at the top right.
Search for the patient by name or phone number and select their profile.
Send the Forms:
After selecting the patient, you will see an option to text them. Look for the "Send General / Consent Form" button at the bottom right of the panel.
A new window will open where you can:
View pending forms for the patient.
Select additional forms to send.
Choose how and to whom the forms will be sent.
Sending Forms via the Patient Information Window
Search for the Patient:
You can find the patient using the search function at the top right of the website. Then, click their name to open the Patient Information Window.
Access Forms:
In the Patient Information Window, navigate to the "Form" tab. A pop-up will allow you to select the forms to send the patient.
Send the Forms:
Add additional forms or consents or remove any recommended forms that you do not want to send the patient.
Follow the same steps described for the Communication Center to complete the process.
Custom Tags in Consent Forms
Some consent forms require Custom Tags, which are additional details you must input before sending the form. For example, if the form requests tooth numbers or specific notes, these fields must be filled out.
Filling in Custom Tags:
Locate fields with outlined icons that require input.
Click on the icon and enter the required information.
Ensure all Custom Tags are completed before clicking "Send."
Note: Forms cannot be sent until all required Custom Tag fields are filled out.
