Practice by Numbers offers an intelligent automation feature called PBN AI Report Summary. This tool utilizes AI to automatically summarize patient-submitted forms, enabling providers to quickly grasp key information without needing to read the entire document. It’s especially helpful for reviewing medical and dental history forms.
⚠️ Important: These settings are part of Forms and are not related to the optional AI Add-on features. They are automatically included with your Forms module at no additional cost.
For practices integrated with Open Dental, you can configure two main things:
Whether AI-generated summaries should be automatically saved into the patient chart.
Which type of summary format you prefer when generating summaries.
How to Find and Update the Settings
Log in to your Practice by Numbers account.
Select Forms at the top of the page, followed by Settings.
Scroll down until you see the section titled Medical History Forms and Dental History Forms.
For each section, you’ll see:
A checkbox to control whether summaries are saved into Open Dental.
Radio buttons to select the Preferred Form Summary Generation Method (AI Summary, Short Label Summary, or Full Label Summary).
Update your preferences by checking/unchecking the boxes and selecting your preferred summary format.
Click Save at the bottom of the page.
💡 Tip: You can configure Medical and Dental History separately, so if you want summaries saved for one form type but not the other, you can set them differently.
What This Setting Does
You’ll see two checkbox options in your system settings:

These checkboxes determine whether the PBN AI Report Summary is pushed into Open Dental.
When the Checkbox Is Checked
The AI-generated summary from the submitted form will automatically overwrite the existing entry in the designated Open Dental section.
Every time a patient fills out a Medical or Dental History form, the corresponding summary will be replaced with the most recent AI-generated version.
When the Checkbox Is Unchecked
The AI-generated summary will not update Open Dental.
You can still view the summarized content within Practice by Numbers, but no data will be written to the Medical Summary or Service Notes sections in Open Dental.
This effectively disables automatic AI chart updates for those forms.
Preferred Form Summary Generation Method
Below the Open Dental checkbox, you’ll see a setting for Preferred Form Summary Generation Method. This allows you to decide how detailed or concise the summaries should appear.
You can choose from three options:
AI Summary (Default)
Uses natural language to create a short, readable narrative of the patient’s responses.
Highlights important conditions, medications, allergies, or other key details.
Best for providers who want a quick, human-readable overview.
Example:
"Patient reports taking Lisinopril daily for hypertension, has seasonal allergies, and no history of diabetes or surgeries."
Short Label Summary
Creates a condensed, structured list of responses.
Less descriptive than AI Summary, but easier to scan quickly.
Useful if you want a concise reference without full sentences.
Example:
Medications: Lisinopril
Conditions: Hypertension, Seasonal allergies
Surgeries: None
Diabetes: No
Full Label Summary
Produces a comprehensive, detailed summary including all form responses with labels.
More verbose, closer to the raw form data, but still structured for clarity.
Ideal for practices that want every detail preserved in the chart.
Example:
Current Medications: Lisinopril (daily)
Medical Conditions: Hypertension, Seasonal Allergies
Diabetes: Patient denies history
Surgeries: None reported
Family History: Not significant
Why Use AI Form Summaries?
Saves time: Providers don’t need to read through the entire form.
Highlights important information: Brings health conditions, medications, and allergies to the forefront.
Improves efficiency: Summaries are immediately accessible during appointments or reviews.
Best Practices
✅ Enable the checkboxes if:
You want patient-submitted history forms to be automatically summarized and saved into Open Dental.
You trust AI to consistently update the patient record with the most relevant information.
❌ Leave the checkboxes unchecked if:
You prefer to review each summary manually before it enters the patient chart.
You do not want summaries to automatically overwrite existing chart data.
