Full Name of the patient
New Patient Identifier (“NP”)
Date of Birth (DoB)
Together, these appear in a clear format like:
John Doe (NP, DoB 07/25)

How It Works
Form Submission: When a new patient fills out and submits an online form (e.g., Medical History, Insurance Info), their name and date of birth are captured automatically.
Information Displayed: The system presents this data using the following format:
Patient Name (NP, DoB MM/DD)
Example: John Doe (NP, DoB 07/25)
John Doe = Patient’s full name
NP = “New Patient” tag
DoB 07/25 = Patient’s date of birth (July 25)
