In Practice by Numbers, you can streamline your workflow by grouping multiple patient forms and consents into Form Packages. These packages allow you to send or assign multiple forms to a patient at once—saving time and ensuring consistency.
Accessing Form Packages
Go to the Forms tab at the top of the page.
From the left-hand menu, select Packages.
Default Package: New Patient Package
By default, Practice by Numbers provides a New Patient Package.
If the New Patient Automation is enabled, this package is automatically sent to new patients.
It includes essential forms and consents typically needed for onboarding.
🔹 Important: This is the only package automatically sent via automation. Any additional packages you create must be sent manually.
Creating or Editing a Form Package
You have two options:
Edit the existing New Patient Package.
Create a new custom package for other patient types or purposes.
To create or customize a package:
Click on the Customize icon (or "Create New").
Enter a Package Name and optional Description.
Select the forms and consents you’d like to include.
Save your package.
Using a Form Package
Once your package is created:
Navigate to the patient profile or appropriate section to send forms.
Click +Form or +Consent.
In the new window, select the Form Packages tab.
Choose your package and click Add.
Select your preferred delivery method (e.g., SMS, Email).
Automation vs. Manual Sending
The New Patient Package is the only package that’s tied to automation (when enabled).
Other packages, like an “Existing Patient Package,” are not automated.
These must be sent manually.
Existing patients will only receive forms based on the frequency settings defined for each individual form.
✅ Example: Creating an "Existing Patient Package" does not mean that all existing patients will automatically receive these forms. Instead, they’ll receive forms only if those forms are set to repeat on a scheduled basis.
